We believe in being transparent in our business and will always do our best to inform you of the cost related to any of our services. We are a fee-for-service practice so payment is due at the time of service. We also require a deposit to hold any appointment and use that as our no show fee.

Rates

New patients receive an introductory package for $400, which includes two comprehensive appointments: an initial 90-minute session and a follow-up visit of up to 60 minutes.

$50 for 15 minutes for follow ups and other concerns for all of our services with Dr. Hanagud & Lynda Kelly

Appointments canceled with less than 24 hours' notice will incur the following cancellation fees:

$25 for follow-up visits $75 for new patient appointments. Please call to cancel your appointment; this courtesy allows other patients to utilize the time.

We take all forms of payment; cash, credit card,check. We also have gift certificates available online for any of our services or retail products.
Have insurance?
Our practice operates on a fee-for-service basis and does not accept insurance directly. We will gladly furnish a detailed invoice with the appropriate billing codes, which you may submit to your insurance company or health plan for reimbursement. Reimbursement policies vary, so we recommend contacting your provider to confirm the details of your coverage.
Important: We are a Medicare opt-out practice, and Medicare will not reimburse for our services."

Appointments
If Gaia Family Medicine owes you a refund due to an overpayment or credit balance, we will issue a refund after our management has verified it. Provided there are no other balances owed to Gaia Family Medicine, or any of its providers, we will credit your credit card or send you a check, depending on how you made your initial payment.

Return Policy for any products
Due to the specific nature of products we offer, we do not accept returns. We do not resell returned items for your protection. No one can guarantee that the storage conditions are met during the order transportation thus we cannot accept them even if the package remained unopened. All supplement sales are final.

Office
No appointment necessary to purchase any of our retail products for healthy living and supplements in the office. You can also drop by to see if any of our wellness services are available.

Cancellation Policy
Our time is valuable and we understand that unforeseen events can happen that prohibits you to keep your appointment, we understand this. However we are a small practice with short windows in order to do business so any cancellation is important to understand that this undercuts our bottom line and we cannot absorb these costs like large medical practices.
For any reason you cancel your appointment with less than 24 hour notice you will be charged a cancellation fee as follows:
$25 for follow-up visits (50%)
$200 for new patient appointments.(50%)
If you cannot make your appointment, please call within 24 hours and inform us so other patients have the opportunity to utilize this time. In some cases cancellation fees can be used as account credit for merchandise and wellness services, not patient appointments.

Shipping
Online items will be shipped within 2-3 days from when order is placed. Shipping is calculated based on zip code. Items should be received within 7 days of shipping unless the item is customized and stated otherwise.
All Online orders will pay an additional 3% convenience fee.
